NURS 6470 - Care Management for Individuals Practicum

The Care Management for Individuals Practicum is the second of two practice for the care management program. In this course, students develop skills to transition to the care manager role for individuals. Students will apply evidence-based didactic knowledge of care for individuals across systems, over a life-span, with attention to disease and wellness planning. Oversight for the practicum experience is provided by the instructor and the designated organizational partners.
