NURS 6390 - Care Management: Clinical Issues

Meets with GERON 6390. This course is the second of two core care management courses. Students will examine interdisciplinary, evidence-based, and outcomes focused care management literature related to care management of a client. Students will develop a comprehensive plan across systems of care specific to their client's disease and wellness trajectories. Communication and collaboration of care management plans across interdisciplinary teams and healthcare systems will also be addressed.
